RN Case Manager/Health Coach

We are seeking a highly skilled RN Case Manager/Health Coach to join our team at Ochsner Health. This individual will be responsible for assessing and managing the bio-psychosocial needs of patients in the ambulatory setting, applying the nursing process to provide quality patient care.
Key Responsibilities- Oversee the disease registry and pre-visit chart review of patients and families on self-management support.
- Coordinate care across the care continuum (case management).
- Assist in developing tools and quality improvement activities.
- Perform other related duties as required.
- Required: Registered nurse diploma.
- Preferred: Associate's degree in nursing.
- Required: 3 years of clinical experience in the delivery of patient care as a registered nurse in a diverse medical background.
- Required: Current registered nurse (RN) license in state of practice.
- Proficiency in using computers, software, and web-based applications.
- Verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the required job processes.
- Good organizational, decision-making, and delegating skills.
- High level of interpersonal and public relation skills.
- Leadership skills.
- Ability to handle high pressure and be highly motivated, eager to learn new skills, and find creative solutions to challenges.
The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste.
