RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Ochsner Health. As a RN Case Manager, you will play a critical role in ensuring that patients receive the necessary care and resources to achieve optimal health outcomes.
Key Responsibilities- Perform pre-admission, admission, and continued stay reviews to ensure that patients receive medically necessary care.
- Collaborate with the care team to develop and implement discharge plans that meet the individual needs of patients.
- Communicate effectively with patients, caregivers, and healthcare providers to ensure seamless care transitions.
- Review patient hospitalizations and recommend a level of care that meets medically necessary hospital care criteria.
- Work as a liaison between ED physicians, hospitalists, attending physicians, and physician advisors to ensure that patients receive the necessary care and resources.
- Current registered nurse license in the state of practice.
- Basic Life Support (BLS) certification from the American Heart Association.
- 3 years of hospital-based experience in discharge planning, case management, or utilization review (preferred).
- Certification in Case Management (CCM) or Accredited Case Manager (ACM) (preferred).
- Bachelor's degree in nursing.
- Proficiency in using computers, software, and web-based applications.
- Effective verbal and written communication skills.
- Strong interpersonal skills.
The RN Case Manager will work in a fast-paced hospital setting, interacting with patients, caregivers, and healthcare providers to ensure seamless care transitions. The ideal candidate will be able to work effectively in a team environment, communicate effectively, and prioritize tasks to meet deadlines.
