Job SummaryWe are seeking a skilled RN Case Manager/Health Coach to join our team. This role will assess and manage the bio-psychosocial needs of patients in the ambulatory setting, applying the nursing process to provide high-quality patient care.
Responsibilities- Conduct pre-visit chart reviews to ensure comprehensive patient care.
- Work with patients and families to develop self-management plans.
- Coordinate care across the care continuum, ensuring seamless transitions and optimal outcomes.
- Assist in developing tools and quality improvement activities to enhance patient care.
- Perform other related duties as required.
- Registered nurse diploma or Associate's degree in nursing.
- 3 years of clinical experience in patient care as a registered nurse in a diverse medical background.
- Current registered nurse (RN) license in the state of practice.
- Associate's degree in nursing.
This role requires the ability to exert 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms, posing an occupational risk for exposure to all communicable diseases.
