LPN - Ambulatory Care Specialist

Actalent is seeking a skilled LPN to join our team as an Ambulatory Care Specialist. In this role, you will provide exceptional patient care and support in a dynamic and fast-paced environment.
Key Responsibilities:- Provide safe and evidence-based care to patients in the Dermatology practice
- Obtain and assess vital signs, patient history, medications, and other pertinent demographic information
- Present patient analysis to physicians using concise and accurate verbal communication
- Assure all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines
- Prepare patients for clinic visits, greet and escort patients into exam rooms
- Set up for surgical procedures and maintain an efficient flow
- Assist surgeon during surgical procedures using clean and sterile techniques
- Communicate the need for follow-up appointments to patients, family members, and staff
- Ensure all follow-up care is accurate and complete
- Receive patient phone calls, triage according to practice policies, and ensure timely response and loop closure
- Provide patient instructions and pre-fill Lido/Epi for overflow, labeled and dated
- Provide date, time, and patient info on all consent forms
- LPN License in the state of CT
- Attendance and reliability
- EPIC experience
- Medical office experience (1-2 years)
