We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Trinity Health. As a key member of our interdisciplinary team, you will provide primary medical management in collaboration with our PACE Organization (PO) Physicians.
Key Responsibilities- Provide primary medical coverage in collaboration with PO physicians as needed.
- Perform initial, semi-annual, and annual assessments, and implement appropriate plans of care to PO participants.
- Evaluate and treat participants during acute illness, prudently prescribing medications, therapies, and other treatments as needed.
- Participate in discharge planning for acute and long-term placement, and make nursing home or home visits as required.
- Work with the Medical Director and Clinic Supervisor to formulate clinical policies, procedures, and standards of care.
- Advise the Medical Director and Primary Care Provider on ways to establish better accountability of PO services to participants and referral sources.
- Master's degree in Nursing.
- Five to seven years of experience in long-term care, preferably.
- Current, unencumbered, active license to practice in the state of PA.
- CDS Certification and DEA Registration required.
- Board Certification by ANCC or AANP.
- Certification as a Nurse Practitioner with prescriptive authority required.
- Current BLS required.
