We are seeking a skilled Hospice Registered Nurse (RN) Case Manager to join our team at Resource 1 Homecare Staffing, LLC. The ideal candidate will have a passion for hospice care and the ability to build relationships with patients and their families.
Responsibilities- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Participate in on-call duties and provide 24/7 support to patients and families.
- Document patient information accurately and thoroughly in our electronic medical record system.
- Stay up-to-date with the latest developments in hospice care and participate in ongoing education and training opportunities.
- Graduation from an accredited school of nursing.
- Current RN licensure in the state of Minnesota.
- 1-2 years of recent acute care experience in an institutional setting.
- 1-2 years of recent home care experience (preferred).
- Valid driver's license and insured automobile.
- Ability to work efficiently with minimal supervision and prioritize tasks effectively.
- Competitive salary range: $75,000 - $90,000 per year.
