We are seeking a skilled Licensed Practical Nurse (LPN) to join our healthcare team at Wellpath. As a key member of our patient care team, you will play a vital role in providing high-quality care to our patients.
Key Responsibilities- Assess patients' physical, psychological, and social dimensions to contribute to individualized treatment planning.
- Implement treatment programs directed by healthcare practitioners, administering medications and performing diagnostic tests as needed.
- Collaborate with healthcare teams to ensure effective patient care and respond to health emergencies within standard guidelines.
- Document nursing encounters using SOAP charting and communicate with nursing staff, physicians, and other relevant personnel.
- Attend mandatory staff meetings and training sessions to demonstrate a commitment to continuous professional development.
- Graduate from an accredited school of nursing.
- Prefer a minimum of one (1) year clinic experience.
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
- Must be able to obtain and maintain CPR certification.
