Job Title: Home Health Nurse LPN

At Aveanna Healthcare LLC, we are seeking a skilled and compassionate Home Health Nurse LPN to join our team. As a Home Health Nurse LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Job Summary:

The Home Health Nurse LPN will work under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient care and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
Work Environment:

The Home Health Nurse LPN will work in a variety of settings, including patients' homes, clinics, and hospitals. The ability to work in a fast-paced environment and adapt to changing situations is essential.
