Job Title: Home Health Nurse LPN

We are seeking a skilled and compassionate Home Health Nurse LPN to join our team at Aveanna Healthcare. As a Home Health Nurse LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document all patient interactions, assessments, and care plans in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
