Responsibilities:
- Provide skilled nursing care to patients in their homes, including assessments, interventions, and education.
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
- Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
- Document all patient interactions, assessments, and interventions in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements:
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
