Key Responsibilities
  • Conduct initial and ongoing supervisory visits to patients' homes to assess their needs and provide training and support to Home Health Aides.
  • Document patient visits and care plans in our electronic medical record system, ensuring timely and accurate charting.
  • Orient and supervise Home Health Aides in the field, providing guidance and support as needed.
  • Collaborate with our clinical care team to ensure seamless care coordination and communication.
  • Develop and implement care plans that meet the unique needs of each patient.
Requirements
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of two years of experience as an RN in a home care setting.
  • Valid driver's license may be required.
