About the Role

CenterWell Home Health is seeking a compassionate and skilled Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in providing personalized care to patients in their homes, helping them to maintain their independence and quality of life.

Key Responsibilities
  • Provide direct patient care under the supervision of a Registered Nurse, following a personalized care plan.
  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Help patients with medication management, including opening bottle caps and reading labels.
  • Take vital signs, such as temperature, pulse, and blood pressure, and report any abnormalities to the supervisor.
  • Assist patients with mobility, exercises, and transfers, as per the care plan.
  • Document interactions with patients, caregivers, and healthcare professionals, ensuring continuity of care.
Requirements
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation.
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
