Job Summary

We are seeking a skilled and compassionate Healthcare Coordinator to join our team at Selfhelp Community Services. As a key member of our healthcare team, you will provide one-on-one health coordination and support to older adults residing within a community-based setting.

Key Responsibilities
  • Schedule medical appointments and ensure timely follow-up
  • Help identify and schedule available transportation for clients
  • Provide advocacy and support during medical appointments
  • Act as primary liaison to medical providers, families, advocates, and other service providers
  • Reconcile medication with provider and client
  • Monitor outcomes of appointments and encourage completion of necessary medical follow-up
  • Explain, educate, and encourage clients in developing a healthier lifestyle
  • Collaborate with and refer to social work when psycho-social needs or services are identified
  • Provide home visits to assist with health education
  • Maintain accurate, timely documentation
  • Perform other duties as assigned by supervisor
Requirements
  • LPN License and New York State Certification required
  • One year of experience working in the health care field preferred, with experience in community healthcare settings
  • Knowledge and experience with geriatrics, intellectual and developmental disabilities preferred
  • Must be able to conduct home visits
  • Working knowledge of Microsoft Office Suite and other technology
  • Excellent communication and listening skills
  • Excellent customer service skills with a focus on treating clients with respect and dignity
  • Able to multi-task and work independently with a great attention to detail
