Position Overview
As a Hospice Care Assistant, you will play a vital role in delivering compassionate patient care under the guidance of licensed healthcare professionals. Your responsibilities will include addressing patient needs and ensuring comfort during their care journey.

Key Responsibilities
- Provide essential patient care activities such as:
- Assisting with daily living tasks
- Monitoring vital signs
- Collecting specimens
- Facilitating patient transportation
- Ensure cleanliness and organization of equipment and patient areas
- Deliver direct care to hospice patients, including:
- Bathing and dressing
- Changing linens
- Communicate any changes in patient condition to the RN Case Manager

Qualifications
- High School Diploma or equivalent preferred
- Current certification through the Texas Nurse Aide Registry
- One year of relevant experience in a hospital or nursing home setting preferred
- Must possess American Heart Association BLS for Healthcare Provider certification
- Experience in hospice or home health care is advantageous
- Reliable transportation and valid driver's license are required
