Home Care Coordinator

We are seeking a skilled Home Care Coordinator to join our team at Comprehensive Home Health Solutions. As a key member of our care team, you will be responsible for managing and overseeing the daily operations related to client care within a non-medical home care environment.
Key Responsibilities- Client Intake & Assessment
- Conduct initial intake assessments with potential clients and their families.
- Assess clients' care needs and develop personalized care plans.
- Maintain regular communication with clients to assess ongoing needs and adjust care plans as necessary.
- Caregiver Assignment & Scheduling
- Match clients with qualified care providers, ensuring compatibility and continuity of care.
- Coordinate and schedule caregivers for home visits.
- Monitor and supervise care providers to ensure adherence to care plans and delivery of high-quality services.
- Client Relations
- Act as the primary point of contact for clients and their families, addressing any concerns or questions.
- Perform routine quality checks, including regular phone calls and home visits, to ensure client satisfaction.
- Provide timely follow-up on client inquiries and concerns to ensure a high level of customer service.
- Caregiver Support & Management
- Provide ongoing support to caregivers, offering guidance and resolving scheduling or performance issues.
- Work with the HR department to ensure caregivers meet all compliance and certification requirements.
- Assist in training and onboarding new caregivers as needed.
- Documentation & Compliance
- Maintain detailed client records, including care plans, progress notes, and communication logs.
- Ensure all documentation complies with company policies, local and federal regulations, and privacy laws.
- Monitor and report any changes in clients' health status or care needs.
- Coordination of Services
- Collaborate with other care providers, such as healthcare professionals, to ensure a holistic approach to care.
- Arrange for additional services, such as transportation, companionship, or meal delivery, as needed.
- Quality Assurance & Improvement
- Develop and implement quality control processes to ensure excellent client care.
- Conduct regular quality assurance assessments to evaluate the effectiveness and appropriateness of care services.
- Address any issues or deficiencies identified through quality assurance assessments promptly and effectively.
- Participate in ongoing training and development to stay current with best practices in home care services.
- Education
- High school diploma or equivalent required; Bachelor's degree in healthcare, social work, or a related field preferred.
- Experience
- Previous experience in home care, healthcare, or customer service, particularly in a coordinator or supervisory role.
- Skills
- Strong communication and interpersonal skills.
- Excellent organizational and multitasking abilities.
- Problem-solving skills and the ability to work well under pressure.
- Proficiency with scheduling software and basic office technology.
- Familiarity with Local and Federal Regulations

- Ability to Work Flexible Hours
- Ability to work flexible hours, including evenings or weekends, as needed.
