Job Overview

We are seeking a skilled Home Care Coordinator to join our team at Comprehensive Home Health Solutions. As a key member of our care team, you will be responsible for managing and overseeing the daily operations related to client care within a non-medical home care environment.

Key Responsibilities
  • Client Intake & Assessment
    • Conduct initial intake assessments with potential clients and their families.
    • Assess clients' care needs and develop personalized care plans.
    • Maintain regular communication with clients to assess ongoing needs and adjust care plans as necessary.
  • Caregiver Assignment & Scheduling
    • Match clients with qualified care providers, ensuring compatibility and continuity of care.
    • Coordinate and schedule caregivers for home visits.
    • Monitor and supervise care providers to ensure adherence to care plans and delivery of high-quality services.
  • Client Relations
    • Act as the primary point of contact for clients and their families, addressing any concerns or questions.
    • Perform routine quality checks, including regular phone calls and home visits, to ensure client satisfaction.
    • Provide timely follow-up on client inquiries and concerns to ensure a high level of customer service.
  • Caregiver Support & Management
    • Provide ongoing support to caregivers, offering guidance and resolving scheduling or performance issues.
    • Work with the HR department to ensure caregivers meet all compliance and certification requirements.
    • Assist in training and onboarding new caregivers as needed.
  • Documentation & Compliance
    • Maintain detailed client records, including care plans, progress notes, and communication logs.
    • Ensure all documentation complies with company policies, local and federal regulations, and privacy laws.
    • Monitor and report any changes in clients' health status or care needs.
  • Coordination of Services
    • Collaborate with other care providers, such as healthcare professionals, to ensure a holistic approach to care.
    • Arrange for additional services, such as transportation, companionship, or meal delivery, as needed.
  • Quality Assurance & Improvement
    • Develop and implement quality control processes to ensure excellent client care.
    • Conduct regular quality assurance assessments to evaluate the effectiveness and appropriateness of care services.
    • Address any issues or deficiencies identified through quality assurance assessments promptly and effectively.
    • Participate in ongoing training and development to stay current with best practices in home care services.
Requirements
  • Education
    • High school diploma or equivalent required; Bachelor's degree in healthcare, social work, or a related field preferred.
  • Experience
    • Previous experience in home care, healthcare, or customer service, particularly in a coordinator or supervisory role.
  • Skills
    • Strong communication and interpersonal skills.
    • Excellent organizational and multitasking abilities.
    • Problem-solving skills and the ability to work well under pressure.
    • Proficiency with scheduling software and basic office technology.
  • Familiarity with Local and Federal Regulations
    • Familiarity with local and federal regulations regarding home care services.
  • Ability to Work Flexible Hours
    • Ability to work flexible hours, including evenings or weekends, as needed.
