Key Responsibilities:
  • Provide high-quality patient care in a home setting
  • Collaborate with our interdisciplinary team to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Administer medications and treatments as prescribed by physicians
  • Document patient progress and communicate effectively with caregivers and family members
Requirements:
  • Current LVN license in good standing
  • 1 year of experience in an acute care or equivalent setting
  • CPR certification with American Heart Association or American Red Cross
  • Positive attitude and dedication to quality patient care
  • Strong communication and interpersonal skills
Preferred Qualifications:
  • 1 year of Home Health experience with adults and geriatrics
  • HCHB and/or Point Click Care experience
