Job Summary:

As a Clinical Social Worker - Case Manager, you will play a vital role in collaborating with the healthcare team to coordinate appropriate services for members of a defined population. Your goal will be to help achieve their optimal level of independence and enhance their quality of life.



Key Responsibilities:
  • Identify and assess the needs of members, providing problem-solving and psychosocial support.
  • Develop and implement care plans to address the unique needs of each member.
  • Collaborate with healthcare professionals to ensure seamless coordination of services.
  • Provide financial counseling and referrals to community resources as needed.
  • Guide members through health-related legal processes and offer consultation and support to other healthcare professionals.

Requirements:
  • Minimum three years of clinical experience and two years in case management required.
  • Minimum one year of experience with the defined population required.
  • Masters degree in social work (MSW) required.
  • Licensed Clinical Social Worker - Certified (Maryland) within six months of hire.
  • Independent Clinical Social Worker License (District of Columbia) within six months of hire.
  • Licensed Clinical Social Worker (Virginia) within six months of hire.
  • Case Manager Certificate within 36 months of hire.

Preferred Qualifications:
  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
