LPN Residential Services Nurse

We are seeking a skilled LPN to join our team at ServiceNet, providing exceptional care to individuals with developmental disabilities and brain injuries in our residential programs.
Job Summary:The successful candidate will be responsible for monitoring and coordinating healthcare services, advocating for the medical needs of our clients, and collaborating with our multidisciplinary team to ensure optimal health outcomes.
Key Responsibilities:- Monitor and coordinate healthcare services for participants, including follow-up and communication with medical providers.
- Institute appropriate nursing interventions and document all changes in the individual's care plan.
- Maintain current medication and treatment orders, noting any side effects or adverse reactions.
- Assist in training staff on personal care, positioning, feeding, and use of supportive devices.
- Accompany patients to medical appointments and Day Programs.
- High School Diploma or GED required.
- College Degree or Certificate preferred.
- Valid MA Nursing License.
- Valid driver's license and acceptable driving record.
- Physical ability to perform the requirements of the position.
- Good communication and computer skills.
- $68,640-$79,040 salary range based on experience and education.
- Additional sign-on bonus up to $5,000.
