Key Responsibilities
  • Assessment and Care Planning: Analyze patient assessment data, evaluate their condition, and collaborate with healthcare providers to develop and maintain individualized care plans.
  • Patient Education: Provide education to patients, families, and staff members on various healthcare topics, under the direction of healthcare providers or managers.
  • Care Coordination: Coordinate patient care with other healthcare entities, as outlined in their care plan.
  • Medication Administration: Administer medications safely, in accordance with organizational policies and procedures, as well as state requirements.
  • Leadership and Mentorship: LPNs in lead positions serve as subject matter experts, clinical mentors, and professional role models within the practice.
Requirements
  • Licensure and Certification: Current LPN licensure from the state of employment or LPN applicant status (required). BLS certification is preferred upon hire and required within 30 days of independent patient care.
  • Education and Experience: 1 year of clinical patient care experience (preferred). EPIC Electronic Health Record (EHR) training is also preferred.
