Job Summary

The RN Case Manager will be responsible for assessing patient needs, developing personalized care plans, and collaborating with the interdisciplinary team to ensure seamless delivery of services.

Responsibilities:

  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify patients' physical, psychosocial, and environmental needs, reassessing as necessary, no less than every 15 days.
  • Initiate and coordinate care plans, documenting problems, goals, interventions, and patient response.
  • Collaborate with patients, families, physicians, and IDG members to provide daily care.
  • Instruct patients and families in self-care techniques when appropriate.
  • Maintain accurate clinical notes regarding patients' conditions.

Requirements

  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to dependable transportation or public transit.

Estimated Salary: $65,000 - $80,000 per year
