Job Title: Medication History Specialist

Kaleida Health is seeking a skilled Medication History Specialist to support accurate medication reconciliation for new patients admitted at their hospital site.

The successful candidate will assist the entire patient care team in ensuring the completion and accuracy of medication histories. This will involve collecting information from various sources, including the patient, patient caregiver, health information exchange(s), retail pharmacies, physician offices, and other sources as directed by the pharmacist, nurse, provider, or supervisor.

Key Responsibilities:

  • Collect and document accurate medication histories for new patients
  • Assist the patient care team in ensuring the completion and accuracy of medication histories
  • Collaborate with healthcare professionals to ensure accurate medication documentation
  • Communicate effectively with patients, caregivers, and healthcare professionals to ensure accurate medication information

Requirements:

  • HS Diploma or GED required
  • AAS in Business/Healthcare-related degree preferred
  • BA/BS in Business/Healthcare-related degree preferred
  • 3 years of experience in a healthcare setting (within the past 5 years) preferred
  • 1 year of direct patient care in a healthcare setting preferred
  • Typing skills assessment required

Preferred Qualifications:

  • Certified Pharmacy Technician (PTCB certification)
  • Certified Medical Assistant
  • Relevant NYS Healthcare License (such as but not limited to LPN)
  • Actively enrolled in one of the following healthcare degree programs at an accredited institution: Nursing (Nurse/Nurse Practitioner), Pharmacy (Pharmacist), Medicine (Physician), Physician Assistant, Dentistry (Dentist), Dietitian/Nutritionist, Physical Therapist

Work Schedule:

Full-time position with a schedule of 11a-7p, 12-8p, 1p-9p, 2p-10p, and 3p-11p

Salary Range:

$23.76-$30.08 per hour, determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement
