  • Proactively engage members to manage their health and healthcare using Motivational Interviewing Skills.
  • Manage transitions of care for members upon discharge from in-patient hospital stays and emergency rooms.

  • Support members in improving their wellbeing by staying out of the hospital and attending regular visits to their primary physician and specialists.
  • Identify and initiate referrals for social service programs, including financial, community resources, medical, behavioral health, psychosocial, community, and state supportive services.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for members as needed to ensure access to care.
  • Provide clinical support for non-clinical staff, educating them on medications, diagnoses, and disease-specific education.
  • Collaborate with the interdisciplinary team to ensure appropriate services and identify needs.
