Home Health Occupational Therapist

We are seeking an experienced Occupational Therapist to join our team in York, PA. This is a full-time opportunity performing home health visits for adult and geriatric patients.
About the RoleThis position involves making home visits to clients, assessing their functional status, and developing individualized intervention plans to achieve favorable outcomes. You will work closely with a multidisciplinary healthcare team to provide rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Duties and Responsibilities- Making home visits to clients in designated geographic territories
- Performing thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance
- Assessing cognitive, psychological, and motor aspects of performance and developing individualized intervention plans to achieve favorable outcomes
- Evaluating home environment for hazards or barriers to more independent living
- Identifying equipment needs and environmental adaptations resulting in enhanced outcomes for clients
- Continually assessing and revising the occupational therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate
- Educating and instructing patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living
- Accurately documenting observations, interventions, and evaluations pertaining to patient care management and services provided
- A current PA Occupational Therapist license
- Prior home care experience preferred but not required
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills, organizational skills, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
The estimated salary for this role is $80,000-$100,000 per year, depending on experience.
