Home Health Occupational Therapist Specialist

We are seeking a skilled and dedicated Occupational Therapist to join our team at LHC Group in York, Pennsylvania. This is a full-time position that requires a commitment to providing high-quality patient care.
About the Role:
- The Occupational Therapist will work with patients in their homes, assessing and evaluating their functional status, activities of daily living, fine motor coordination, and other occupational therapy needs.
- This role involves conducting thorough assessments, developing individualized treatment plans, and implementing evidence-based interventions to promote patient independence and well-being.
- The Occupational Therapist will work collaboratively with healthcare teams to ensure comprehensive patient care and optimal outcomes.
- Excellent communication and interpersonal skills are essential for this role, as the Occupational Therapist will interact regularly with patients, families, and healthcare professionals.
Responsibilities:
- Evaluate patients' functional status, identify areas of need, and develop tailored treatment plans.
- Implement therapeutic interventions, including exercises, education, and training, to enhance patients' functional abilities and quality of life.
- Maintain accurate records of patient progress, observations, and outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Stay up-to-date with industry developments, best practices, and research findings to maintain expertise and advance professional growth.
Requirements:
- Current Occupational Therapy licensure in the state of Pennsylvania.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.
- Experience working in home health settings preferred but not required.
