Key Responsibilities

The Home Health Patient Care Manager is accountable for overseeing and coordinating clinical services, as well as providing and directing nursing care to patients in their homes as prescribed by the physician and in accordance with relevant laws, regulations, and agency policies.
  • Delivers clinical services within the scope of practice as defined by state laws governing nursing, in alignment with the care plan, and in collaboration with other healthcare team members.
  • Handles referrals, ensures suitable clinician assignments, evaluates patient orders, and schedules start of care visits.
  • Coordinates the assessment of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensures continuous assessment of patient needs and that care provided is tailored to individual requirements, appropriate, meets home health eligibility criteria, and aligns with physician orders.
  • Reviews assessments and care plans daily, according to assigned workflows, and consults with clinicians for recommendations as necessary.

Qualifications

  • Current RN licensure in the state of MA
  • Current CPR certification is required.
  • Valid Driver's License, vehicle insurance, and access to reliable transportation or public transit
