Key Responsibilities
The Home Health Patient Care Manager oversees and coordinates clinical services, directing nursing care for patients in their homes as prescribed by physicians and in accordance with relevant laws, regulations, and agency policies.
- Delivers clinical services within the defined scope of practice, following state nursing laws, the care plan, and in collaboration with other healthcare professionals.
- Handles referrals, ensures suitable clinician assignments, evaluates patient orders, and schedules start of care visits.
- Coordinates assessments of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Continuously assesses patient needs, ensuring that care is tailored to individual requirements, appropriate, meets home health eligibility criteria, and aligns with physician directives.
- Reviews assessments and care plans daily, following assigned workflows, and consults with clinicians for recommendations as necessary.
- Active RN licensure in the state of MA
- Current CPR certification is mandatory.
- Valid Driver's License, vehicle insurance, and access to reliable transportation or public transit
