Job Title: Advanced Practice Provider

We are seeking a highly skilled Advanced Practice Provider to join our team at UBMD Primary Care. As a key member of our healthcare team, you will work with patients on the Chemical Dependency unit, providing medical care and support on an as-needed basis on weekends only.

Job Summary:

The Advanced Practice Provider will work closely with patients, healthcare providers, and other team members to provide comprehensive care and support. This includes performing physical exams, ordering tests, diagnosing and treating substance use disorders, and developing medical management plans.

Responsibilities:
  • Perform medical history and physical examinations, and interpret results of diagnostic studies
  • Develop medical management plans with collaborating physicians and order appropriate therapy and diagnostic tests
  • Re-evaluate patient clinical courses and interpret additional diagnostic tests
  • Evaluate test findings, make initial medical diagnoses, and initiate appropriate action to facilitate therapeutic plans
  • Document clinical information in the electronic medical record, including history and physical, discharge summaries, and consultation reports
  • Apply knowledge of coding to EMR documentation to ensure adequate billing for services
  • Coordinate inpatient medical management and discharge planning with psychosocial care rendered by counselors
  • Participate in design and implementation of treatment protocols for patients managed by collaborating physicians
Requirements:
  • Graduate of accredited Physician Assistant or Nurse Practitioner program
  • Current NYS license as Physician Assistant or Nurse Practitioner certified by the NYS Education Department
  • Minimum of two years' experience in healthcare delivery, preferably in an inpatient setting
  • Experience in providing care and services to patients with substance use disorders preferred
Working Conditions:

The position is in a well-lit, fast-paced, clean hospital environment. Office noise level will be moderate most times. Moderate/average indoor temperatures.
