Job Summary

As a Clinical Value Associate at Penn State Health, you will play a vital role in supporting Population Health activities. Your primary responsibility will be to enhance the patient experience, improve quality, reduce costs for patients' care, and improve care transitions. You will work closely with the Population Health team to develop and implement strategies to improve patient outcomes and reduce healthcare costs.

Key Responsibilities

Track and analyze quality metrics and closing care gaps
Support clinicians in achieving quality incentives through patient outreach (telephonic and electronic outreach)
Maintain accurate and timely documentation in the EMR system and provide regular feedback to patients' PCPs
Identify different panels of patients and reviews reports related to chronic disease management and high-risk patients to assist in chronic care management
Act as a navigator for assigned patients and facilitate communication amongst other providers caring for the patient
Develop collaborative partnership with providers caring for the patient through frequent telephonic and/or in person interactions with the provider and office staff
Coordinate patient visits with PCP and specialists as needed

Requirements

High School Diploma or equivalent required
Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90-days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required
