Job Summary

The Population Health Clinical Value Associate - Medical Assistant is an integral part of the Population Health team, supporting activities that enhance the patient experience, improve quality, reduce costs, and improve care transitions. This role requires adjusting interventions to be sensitive to different cultural, religious, economic, and social groups and how it affects the knowledge, delivery, and acceptance of healthcare services.

Key Responsibilities

Track and analyze quality metrics and closing care gaps.
Supports clinicians in achieving quality incentives through patient outreach (telephonic and electronic outreach).
Maintain accurate and timely documentation in the EMR system and provide regular feedback to patients' PCPs.
Works to identify different panels of patients, and reviews reports related to chronic disease management and high-risk patients to assist in chronic care management.
Act as a navigator for assigned patients and facilitate communication amongst other providers caring for the patient.
Develop collaborative partnership with providers caring for the patient through frequent telephonic and/or in person interactions with the provider and office staff.
Coordinate patient visits with PCP and specialists as needed.

Requirements

High School Diploma or equivalent required.
Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90-days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required.
