Medical Assistant

Job Summary:
Penn State Health is seeking a Medical Assistant to support Population Health activities. The ideal candidate will have a strong understanding of healthcare quality programs, including Medicare, HEDIS Metrics, and Merit-Based Incentive Program (MIPS). They will be responsible for tracking and analyzing quality metrics, closing care gaps, and supporting clinicians in achieving quality incentives through patient outreach. The successful candidate will also maintain accurate and timely documentation in the EMR system and provide regular feedback to patients' PCPs.
Key Responsibilities:
- Track and analyze quality metrics and closing care gaps
- Support clinicians in achieving quality incentives through patient outreach
- Maintain accurate and timely documentation in the EMR system
- Provide regular feedback to patients' PCPs
- Identify different panels of patients and review reports related to chronic disease management and high-risk patients
- Act as a navigator for assigned patients and facilitate communication amongst other providers caring for the patient
- Develop collaborative partnerships with providers caring for the patient through frequent telephonic and/or in-person interactions
- Coordinate patient visits with PCP and specialists as needed
Requirements:
- High School Diploma or equivalent required
- Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90 days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required
