Job Summary

We are seeking a highly skilled and compassionate Residence Counselor/Medical Coordinator to join our team at Constructive Partnerships Unlimited. As a key member of our interdisciplinary team, you will provide exceptional care and support to individuals with developmental disabilities, ensuring their physical, emotional, and social well-being.

Responsibilities
  • Assist individuals with activities of daily living, including toileting, bathing, dressing, and meal preparation.
  • Participate in in-house recreational activities and accompany individuals to community-based programs.
  • Collaborate with the interdisciplinary team to evaluate and implement individualized programs, ensuring effective communication and goal achievement.
  • Maintain accurate records, charts, and logs for each individual, including vital signs, medication administration, and behavioral data.
  • Prepare and serve meals, ensuring individualized dietary needs are met.
  • Perform general housekeeping tasks, maintaining a clean and safe living environment.
  • Administer medications and treatments, recording same and relaying pertinent medical information to the registered nurse.
  • Accompany individuals to medical appointments, outside programming, and recreational activities.
  • Attend required in-service trainings and staff meetings, completing CPR/First Aid and SCIP-R certifications as necessary.
  • Report incidents and medical issues to the Residence Manager and registered nurse, following established protocols.
Requirements
  • High school diploma or GED and prior experience working with individuals with developmental disabilities.
  • Proficient in Microsoft Word and capable of learning new software applications.
  • Excellent organizational skills and ability to communicate effectively in English.
  • Ability to travel to and access multiple locations throughout the metropolitan area.
  • Valid NYS driver's license and proof of Department of Motor Vehicles driving history.
Physical Requirements
  • Ability to coordinate movement and exert physical effort over long periods, including standing for two hours at a time.
  • Ability to lift individuals safely and perform two-person transfers.
  • Ability to push/pull, turn, and maneuver equipment, including occupied wheelchairs.
  • Ability to operate a 15-person van, as required.

This position requires working in a fast-paced, dynamic environment, with frequent interactions with individuals, families, and healthcare professionals. The successful candidate must be able to work effectively in a team setting, prioritize tasks, and maintain a high level of professionalism and confidentiality.
