The navigator identifies and addresses barriers to care, assists with access challenges, and fosters relationships with healthcare providers. They explain insurance and payment systems while coordinating services among providers, patients, and caregivers to ensure a seamless and integrated care experience.
Additionally, the navigator raises awareness about healthcare resources, Sutter Health support programs, community services, and clinical trials, addressing various illness-related needs as identified by patients and their support networks.
Key Responsibilities:
- Conduct ongoing evaluations and contribute to quality improvement initiatives.
- Engage in community outreach and marketing efforts.
- Provide education and support regarding healthcare systems and available resources.
Qualifications:
Education requirements include a Bachelor's degree in Social Work, Health Education, or a related field, or equivalent experience. Alternatively, candidates may have graduated from an accredited nursing program.
Certifications:
Required certifications include RN-Registered Nurse of California, LCSW-Licensed Clinical Social Worker, RCP-Respiratory Care Practitioner, PT-Physical Therapist, or OT-Occupational Therapist. Basic Life Support (BLS) certification is also necessary. Depending on the focus area, current breast patient navigator certification may be required within the first year of employment.
Experience:
A minimum of 5 years of relevant experience is expected.
Skills and Knowledge:
Candidates should demonstrate proficiency in evidence-based practices and possess in-depth knowledge of the focus area, including treatment options and potential complications. Strong analytical and project management skills are essential, along with the ability to communicate clinical information effectively to diverse audiences.
Proficiency in Microsoft Office Suite and familiarity with electronic health record systems are required. The ability to work independently and as part of a team while maintaining exceptional attention to detail is crucial.
Work Schedule:
This is a full-time position with a standard workweek of Monday through Friday, requiring 40 hours per week.
