We are seeking a skilled Licensed Vocational Nurse to join our team as a Care Coordinator. In this role, you will work under the direct supervision of the Population Health Director of Care Management or Nurse Manager to provide high-quality patient care.
Key Responsibilities- Facilitate communication between patients, caregivers, physicians, and the outpatient care team to optimize health and wellness.
- Identify and manage patients with high-risk disease management and complex comorbid conditions.
- Work collaboratively with Nurse Navigators to manage medical stability and close care gaps.
- Schedule preventative services and document encounters.
- Employ knowledge of evidence-based guidelines to communicate effectively with patients and care teams.
- Maintain confidentiality and adhere to regulatory requirements.
- LVN License in the state of employment or compact.
