About the Role

The RN Care Coordinator will be responsible for providing comprehensive care to pediatric transplant patients in both inpatient and outpatient settings. They will work closely with transplant surgeons and physicians, as well as other healthcare professionals, to deliver exceptional patient outcomes.

The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. They will also have a strong understanding of transplant care and a passion for delivering high-quality patient care.

Responsibilities
  • Provide comprehensive care to pediatric transplant patients
  • Work closely with transplant surgeons and physicians to develop treatment plans
  • Collaborate with other healthcare professionals to deliver exceptional patient outcomes
  • Manage and supervise operational functions and activities of the transplant programs
