Job Description:

Navi Nurses seeks a skilled Care Coordinator Specialist to join our team. As a key member of our operations, you will play a vital role in ensuring the highest quality of service for each client's co-created care plan.

The ideal candidate will have a robust clinical background and a passion for delivering patient-centered care. Your strong interpersonal and communication skills will enable you to build trust with clients and navigate their complex needs effectively.

Responsibilities:
  • Provide exceptional customer experiences by anticipating client needs and addressing concerns proactively.
  • Demonstrate commitment to delivering high-quality care through seamless coordination with Navi Nurses administrative team and nurse contractors.
  • Balancing responsibilities across client management, clinical assessments, client advocacy, nursing support, guiding care planning, data reporting, and clinical operations.


Requirements:
  • A registered nurse (RN) with 4+ years of experience.
  • An Arizona RN license in good standing, unencumbered.
  • 4+ years of direct client care experience (at least 1 year in an acute setting within the last 5 years).
  • Current AHA BLS and/or ACLS certification.
  • ASN or higher & graduated from an accredited nursing program.


This is a full-time position offering a competitive salary range of $45-$50 per hour, with opportunities for performance-based bonuses on a quarterly basis. You will be required to travel up to 25% locally, meeting new and existing clients across Maricopa County as determined by business needs.
