As a Family Practice/Primary Care Nurse Practitioner, you will be responsible for providing high-quality patient care and working collaboratively with our healthcare team to deliver exceptional patient outcomes.
Responsibilities:
- Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough health assessments, develop and implement individualized care plans, and provide education to patients and their families.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
- Stay up-to-date with the latest medical research, technologies, and best practices in family medicine to provide evidence-based care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
Requirements:
- Master's degree in Nursing or related field.
- Current licensure as a Nurse Practitioner in the state of Rhode Island.
- Board certification in Family Medicine or related specialty.
- Minimum of 2 years of experience as a Family Practice/Primary Care Nurse Practitioner.
- Excellent communication and interpersonal skills.
