Pediatric RN Case Management Supervisor

Employment Type: Full Time

Schedule: Monday to Friday, 9 AM - 5 PM

As the Pediatric RN Case Management Supervisor, you will play a pivotal role in overseeing pediatric patient care services. You will report directly to the Director of Patient Services and collaborate closely with the RN Case Manager and other interdisciplinary team members to ensure that care is delivered in compliance with regulatory standards.

Key Responsibilities:

  • Coordinate patient care that is efficient and eligible for reimbursement under the patient's insurance plan, with physician approval.
  • Review insurance details and confirm eligibility with the operations team.
  • Examine patient charts to ensure all services align with the established plan of care.
  • Interpret CHHA reimbursement policies and identify appropriate billing sources for patient care.
  • Ensure clinical documentation supports care coordination and adheres to the plan of care.
  • Engage in Quality Assurance and Performance Improvement (QAPI) initiatives and conduct quality audits to uphold care standards.
  • Participate in staff training programs, including clinical orientation and supervisory field visits.
  • Review documentation to confirm it accurately reflects the patient's clinical status and the necessity for CHHA services.
  • Ensure compliance with service frequencies and durations as specified in physician orders.
  • Convene care coordination meetings to assess service adherence, patient progress, and discharge planning.
  • Maintain regular communication with the RN Case Manager and interdisciplinary team to adjust patient care goals and interventions as necessary.
  • Act as the coordinator for the multidisciplinary team providing patient care.
  • Develop and update care plans with input from relevant health team members, ensuring they meet the needs of the patient and family.
  • Document care coordination efforts in the medical record as required.
  • Supervise pediatric nursing staff, including conducting joint visits and annual evaluations in accordance with Department of Health guidelines.
  • Monitor the impact of care plans on patient health and communicate any changes in needs to the physician and team.
  • Facilitate and document the discharge process for patients from the program.
  • Complete all necessary documentation promptly.
  • Assist in the orientation of new pediatric nursing staff.
  • Share on-call responsibilities for weekends and holidays with other nursing staff.
  • Perform additional duties and projects as assigned, including home care visits and community outreach.

Qualifications:

  • Valid and current Registered Professional Nurse license in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Basic Life Support (BLS) certification required.
  • Minimum of 3 years of experience in a home care or CHHA setting.
  • Supervisory experience is advantageous.
  • Experience with pediatric patients is a plus.
  • Full COVID-19 vaccination is mandatory.
  • Reliable transportation or access to public transit for travel within designated areas.
  • Willingness to work on holidays and weekends as part of a rotating schedule.
