Position Overview

Pediatric RN Case Management Supervisor

Employment Type: Full Time

Schedule: Monday to Friday, 9 AM - 5 PM

Key Responsibilities:

  • Oversee the coordination of pediatric patient care services, ensuring compliance with regulatory standards.
  • Collaborate with the Director of Patient Services and the interdisciplinary team to deliver high-quality care.
  • Review and verify insurance eligibility and ensure that care provided aligns with the patient’s insurance plan.
  • Conduct thorough reviews of patient charts to confirm that all services are delivered according to the established plan of care.
  • Participate in quality assurance and performance improvement initiatives to enhance care delivery.
  • Engage in staff training and development, including clinical orientation and supervision of nursing staff.
  • Maintain accurate documentation reflecting the clinical status of patients and the necessity for services.
  • Facilitate care coordination meetings to assess patient progress and adjust care plans as needed.
  • Communicate effectively with the RN Case Manager and other team members to ensure seamless service delivery.
  • Supervise pediatric nursing staff, including conducting joint visits and performance evaluations.
  • Monitor patient health outcomes and adjust care plans in consultation with physicians.
  • Ensure timely completion of all required documentation and participate in the orientation of new nursing staff.
  • Provide on-call coverage on weekends and holidays as part of a rotating schedule.
  • Engage in community outreach and maintain relationships with hospitals and other healthcare providers.

Qualifications:

  • Current registration and license to practice as a Registered Professional Nurse in New York State.
  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Basic Life Support (BLS) certification required.
  • A minimum of three years of experience in a home care or CHHA setting.
  • Supervisory experience is advantageous.
  • Experience working with pediatric patients is preferred.
  • Full COVID-19 vaccination is mandatory.
  • Reliable transportation is essential for travel within designated areas.
  • Willingness to work weekends and holidays on a rotating basis.
