Nutrition Care Specialist

This position provides comprehensive nutrition care to patients by assessing their nutritional needs, developing and implementing care plans, and monitoring outcomes. The ideal candidate will possess strong communication and leadership skills, with the ability to work effectively in an interdisciplinary team environment.
Key Responsibilities- Conduct comprehensive nutrition assessments of patients' nutritional needs
- Develop and implement individualized care plans to meet patients' nutritional needs
- Monitor and evaluate the effectiveness of care plans and communicate with the healthcare team to ensure optimal nutrition outcomes
- Provide education to patients, families, and healthcare professionals on nutrition and dietetics
- Participate in quality improvement activities to ensure optimal nutrition care outcomes
- Bachelor's degree in Nutrition and/or Dietetics
- Registered Dietitian (RD) or RD Eligible (academic and professional requirements as defined by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics)
- Current registration through the Commission on Dietetic Registration of the American Dietetic Association
- Ability to communicate effectively and function independently
