Home/Community Care Specialist

The Fairbanks Resource Agency Inc is seeking a skilled Home/Community Provider to implement prescribed habilitation plans for eligible participants. This role involves providing routine direct care in the home, community, or center-based settings, working closely with families, guardians, and the FRA Home/Community Services Manager to design and implement consistent intervention strategies.
Key Responsibilities- Deliver direct care services as per the Plan of Care (POC), including meal preparation, eating, toileting, bathing, dressing, and recreational activities.
- Ensure health and safety needs are met, and prepare and/or assist individuals to prepare meals, ensuring nutritional needs are met.
- Observe, supervise, and record participant activities, discussing responses to intervention strategies with the Home/Community Manager and other staff members.
- Maintain records and prepare reports on services provided, including participant information and case notes.
- Perform or request emergency aide as needed, and maintain current knowledge of developmental disabilities.
- Input participant information and case notes into the electronic medical record (Therap), in accordance with agency procedures, state, and federal requirements.
- Attend training seminars, in-service sessions, and staff meetings as requested, and maintain training compliance in accordance with FRA policies and procedures, state, and federal regulations.
- High School Diploma or equivalent.
- Preferred six months of experience working with individuals with developmental disabilities.
- Must meet agency insurance criteria for operating agency and personal vehicles on agency business.
- Demonstrate proficiency in direct care activities.
- Ability to communicate effectively, both orally and written, with participants, families, guardians, supervisor, other agency staff, and the general public.
- Excellent observational and organizational skills to assess and record participant responses to interventions.
- Ability to work flexible schedules, independently, and prioritize activities with minimal direction.
- Ability to be punctual, reliable, enthusiastic, and professional.
- Alaska Driver's License.
- Agency Provided Certifications, including Certification in Adult/Child and Infant CPR, Certification in First Aid, and Safety-Care Certification.
- Annual PPD Test.
- Hepatitis B immunization (or) Waiver.
Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
Physical Activities- Stooping: Bending body downward and forward by bending spine at the waist.
- Crouching: Bending the body downward and forward by bending leg and spine.
- Reaching: Extending hand(s) and arm(s) in any direction.
- Standing: Particularly for sustained periods of time.
- Walking: Moving about on foot to accomplish tasks, particularly for long distances.
- Pulling: Using upper extremities to exert force in order to draw, drag, haul, or tug objects in a sustained motion.
- Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
- Talking: Expressing or exchanging ideas by means of the spoken word.
- Hearing: Perceiving the nature of sounds with no less than a 40 db loss.
- Driving: Minimum standards required by State Law (including license).
None: The worker is not substantially exposed to adverse environmental conditions (such as typical office or administrative work).
