About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Linwood, New Jersey.

Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Physical Therapist with BAYADA, you will be an integral part of a multidisciplinary healthcare team, working collaboratively to provide rehabilitative care and skilled nursing to clients in their own homes.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance assessments, to promote clients' independence and mobility.
  • Collaborate with nursing staff to develop and implement comprehensive care plans that address clients' physical, emotional, and social needs.
  • Provide education and training to clients and their families on rehabilitative care and activities necessary to promote healthy living.
  • Document client progress and outcomes, utilizing a state-of-the-art touch pad tablet.
Requirements
  • A current STATE Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
