Job Summary

We are seeking an experienced Nurse Practitioner or Physician Assistant to join our team as a Community Health Specialist. This field-based position will involve providing high-quality, patient-centered care to individuals in Lancaster and York counties, with a focus on addressing care gaps and promoting optimal health outcomes.

Key Responsibilities:

  • Conduct comprehensive health assessments to identify potential health risks and gaps in care.
  • Develop and implement personalized care plans to address identified needs and promote optimal health outcomes.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.

Requirements and Qualifications:

  • A minimum of two years of Advanced Practice Provider experience is required.
  • The successful candidate must reside in York or Lancaster county or nearby.
