Key Responsibilities:
- Collaborate with healthcare professionals to assess patient needs and develop treatment plans
- Advocate for patients to resolve crises and ensure seamless care transitions
- Investigate cases of child abuse and neglect, and take authorized protective action when necessary
- Intervene in crisis situations to assist patients, families, and caregivers
- Refer clients to community and social service programs to support their well-being
- Coordinate with care managers and I/DD organizations to ensure comprehensive care
- Assist with Premier HealthCare initiatives, including Patient-Centered Medical Home (PCMH)
- Participate in bi-weekly clinical operations meetings and monthly quality improvement meetings
- Complete various forms on behalf of patients and may supervise MSW students and other interns/volunteers
Requirements:
- Masters of Social Work from an institution accredited by the Council on Social Work Education (CSWE) or its equivalent
- Knowledge of medical terminology and understanding of the healthcare team's roles
- Exceptional interpersonal and written communication skills, with strong crisis intervention skills
- LMSW or LCSW preferred, with experience in community facilities and resources for discharge planning
