About the Role

As a Clinical Care Coordinator at In-Clinic Rx, you will play a crucial role in supporting our clinical pharmacists in delivering high-quality patient care.

Job Summary

We are seeking an experienced and detail-oriented individual to join our team as a Clinical Care Coordinator. The successful candidate will work closely with our clinical pharmacists to provide exceptional patient care, manage patient relationships, and maintain accurate records.

Responsibilities
  • Enroll patients in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs
  • Prepare patient charts for upcoming CCM encounters and ensure timely communication with clinical pharmacists
  • Make monthly check-in calls to patients enrolled in CCM and daily compliance calls to patients in RPM
  • Maintain accurate records and communicate effectively with patients, caregivers, and healthcare providers
Requirements
  • Nationally Certified Pharmacy Technician (CPhT) certification or equivalent experience
  • Registration as a pharmacy technician with the North Carolina Board of Pharmacy
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in using EHR systems and software applications
