Job Title: Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team at E-VOLVE CARE.

The ideal candidate will be responsible for providing exceptional patient care, administering medications, and collaborating with healthcare professionals to develop and implement care plans.

Key Responsibilities:
  • Develop and Maintain Nursing Services: Create and implement nursing services objectives, standards of nursing practices, and policy and procedures to ensure high-quality patient care.
  • Collaborate with Healthcare Professionals: Work closely with the Director of Nursing (DON) and other healthcare professionals to develop and implement care plans that meet the unique needs of each patient.
  • Provide Preventative Medicine: Implement preventative medicine practices to promote the health and well-being of patients.
  • Coordinate Nursing Services: Assist in coordinating nursing services with other resident-related services provided within and outside of the facility.
  • Develop Individualized Care Plans: Create individualized care plans for each patient to ensure their unique needs are met.
  • Evaluate Resident Records: Evaluate resident records to ensure accuracy, care plans are current and complete, and patients are receiving optimal nursing care.
  • Monitor Lab Results: Monitor lab results, x-rays, dental, optometry, podiatry services, hearing services, and rehabilitative services with other providers.
  • Effective Communication: Communicate effectively with nursing staff members to ensure seamless care delivery.
  • Conduct Social Service Assignments: Help conduct formal social service assignments to ensure patients receive the support they need.
  • Develop Social Services Plans: Develop, implement, and evaluate an effective social services plan, along with other disciplines, to help patients achieve their goals.
  • Coordinate Discharge Plans: Help formulate and coordinate interdisciplinary discharge plans for patients.
  • Ensure Successful Admissions: Ensure successful new admissions and transfers of patients within the center and to outside agencies.
  • Establish Meaningful Relationships: Establish and maintain meaningful, professional relationships with patients and their families to effectively assist them in achieving their goals.
  • Ensure Accurate Nursing Care Records: Ensure the total nursing care of patients and the accuracy of nursing care records.
  • Follow Individual Care Plans: Ensure individual care plans are followed to ensure patients receive the best possible care.
  • Coordinate Nursing Services: Coordinate nursing services for patients in the nursing home with other resident care services provided within and outside of the nursing home.
  • Communicate with Physicians: Communicate with patients' physicians when there are changes in condition.
  • Act as a Patient Advocate: Act as a patient advocate to ensure their rights and needs are respected.
Requirements:
  • Licensure: Must have an unrestricted license with the State of Minnesota as a Registered Nurse and maintain current unencumbered licensure.
  • Knowledge of Regulations: Thorough knowledge of state and federal regulations regarding long-term care.
  • Safety Habits: Know and practice good safety habits.
  • Communication Skills: Strong communication and interpersonal skills.
  • Ability to Work in a Fast-Paced Environment: Ability to work in a fast-paced environment.
  • Teamwork: Ability to work independently and as part of a team.
Compensation:

$41 per hour
