Your expertise will be instrumental in ensuring that our members receive high-quality, patient-centered care.
Key Responsibilities- Assess and plan individualized care strategies for members with complex medical and behavioral needs.
- Utilize strong communication and interpersonal skills to engage members in their care and promote self-management.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and transitions.
- Develop and implement care plans that address social determinants of health and promote healthy behaviors.
- Provide education and support to members on medication management, disease-specific education, and health literacy.
- Identify and initiate referrals for community resources and social services to support member needs.
- Communicate effectively with stakeholders to ensure timely and coordinated care.
- Current, unrestricted RN license in the state of Kansas.
- 2+ years of experience working in community-based settings, such as acute inpatient care, long-term care, home health, hospice, public health, or behavioral health.
- Intermediate level of proficiency in MS Word, Excel, and Microsoft Outlook.
- Ability to travel up to 75% of the time with reliable transportation and a valid Kansas driver's license.
- Access to a designated quiet workspace in your home with high-speed internet connectivity.
- Bachelor's degree in nursing.
- Experience managing Medicaid and Medicare populations with complex medical and behavioral needs.
- Background in arranging community resources and electronic charting.
