Physical Therapy Services Coordinator

Job Summary:
Christus Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients, evaluating their needs, and developing personalized treatment plans to help them achieve their maximum level of independence.
Key Responsibilities:
- Evaluate Patients: Conduct thorough assessments of patients to identify their physical therapy needs and develop effective treatment plans.
- Develop Treatment Plans: Create personalized treatment plans that address the unique needs and goals of each patient.
- Provide Physical Therapy Services: Deliver high-quality physical therapy services to patients, including exercises, stretches, and other interventions to help them achieve their maximum level of independence.
- Communicate with Patients and Families: Maintain open and effective communication with patients and their families to ensure they understand their treatment plans and progress.
- Collaborate with Healthcare Team: Work closely with other healthcare professionals to ensure comprehensive care and coordination of services.
- Maintain Accurate Records: Maintain accurate and up-to-date records of patient progress and treatment plans.
- Participate in Quality Improvement: Participate in quality improvement initiatives to ensure the highest level of care and service.
Requirements:
- Doctorate of Physical Therapy: A Doctorate of Physical Therapy degree from an accredited program.
- Basic Computer Knowledge: Basic computer skills and knowledge of electronic medical records.
- Effective Communication: Excellent verbal and written communication skills.
- Registration and Certification: Current registration and certification in the state of Texas.
Work Environment:
This is a full-time position in a dynamic and fast-paced healthcare environment. The ideal candidate will be a team player with excellent communication and interpersonal skills.
