Job Description

We are seeking a highly skilled Behavioral Health Consultant to join our team at HealthCore Clinic.

This is a unique opportunity to work with a dynamic and caring team, providing mental health and behavioral health services to individuals, families, and groups.

The ideal candidate will have a Master's degree in Social Work, current licensure in Social Work (LSCSW) within the state of Kansas, and prior experience working as a Social Worker in a clinical setting or FQHC.

The Behavioral Health Consultant will be responsible for assessing the clinical status of patients, working with primary care and other departments to treat and manage patients with chronic emotional and/or health problems, and maintaining patient caseloads in accordance with the needs of the Clinic.

The successful candidate will have advanced working knowledge of behavioral medicine and evidence-based treatments for mental health conditions, as well as the ability to communicate effectively with vulnerable populations.

Key Responsibilities:

Patient Population Management:

  • Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
  • Work with primary care and other departments to treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
  • Consult with patient primary care provider to enhance understanding of the patient, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans.
  • Maintain patient caseloads in accordance with the needs of the Clinic.
  • Offer weekly intake appointment times as identified by the Director of Behavioral Health.

Care Coordination:

  • Use the DSM V and other assessment tools to identify and manage patients with various physical and behavioral health issues.
  • Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
  • Provide skill training through psycho-education and patient education strategies and develop specific behavioral change plans for patients and behavioral health protocols.
  • Provide individual, group, and family therapy meeting the varied needs of the patient.

Self-Management Support:

  • Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
  • Provide self-management support to patients by providing self-management tools and encouraging patients to self-manage and promote healthy behavior changes.

Quality Improvement:

  • Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Meet licensure requirements, training requirements, and develop individual competencies; track all training required in order to keep clinical licensure.

Additional Responsibilities:

  • Provide on-call coverage in rotation with all department clinical staff as needed.
  • Thoroughly prepare for meetings and court appearances and provide court testimony when necessary.
  • Complete documentation in compliance with clinic policy and in compliance with governing regulations.

Qualifications and Requirements:

  • Current and valid licensure in Social Work (LSCSW) within the state of Kansas required.
  • Master's degree in Social Work is required.
  • Practicum clinical experience required.
  • Prior experience working as a Social Worker in a clinical setting or FQHC preferred.
  • Advanced working knowledge of behavioral medicine and evidence-based treatments for mental health conditions.
  • Knowledge of special needs populations including MR/DD and SED children.
  • Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
  • Ability to communicate effectively with vulnerable populations.
  • Ability to maintain absolute confidentiality in regards to HCC matters.
