Responsibilities:
  • Perform medical nutrition therapy assessments to address acute and chronic conditions.
  • Educate residents on therapeutic diets and nutrition interventions to improve outcomes and quality of life.
  • Work collaboratively with nursing, dietary manager, speech language pathologist, physicians, and nurse practitioners to deliver individualized nutritional therapy.
  • Monitor and assess residents with weight changes, enteral feeding, dialysis, and wounds.
  • Participate in resident review meetings to address high-risk residents.
  • Conduct patient nutrition assessments and develop implementation plans for nutrition intervention.
  • Provide nutrition counseling and follow-ups in accordance with patient treatment goals.
Qualifications:
  • Bachelor's degree in Dietetics or related field.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
  • Illinois state licensure as a Dietitian.
  • Familiarity with PointClickCare and MDS process is a plus.
