Job Summary

We are seeking a skilled Social Worker to join our team in Cancer Registry Support Services. As a key member of our oncology program, you will work in partnership with leadership and healthcare professionals to ensure continuity and coordination of care for adult patients and families.

Key Responsibilities
  • Assume a daily case load, identifying and coordinating cancer support resources, discharge planning, and referral to appropriate resources.
  • Provide consultation to oncology clinical staff on the psychosocial aspects of patient care and available services.
  • Assess patients through interviews to gather necessary data about their situation, stresses, functional status, financial system, and support systems.
  • Perform other duties as assigned.
Requirements
  • Masters degree in Social Work.
  • One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW).
  • 2 years of Inpatient, Outpatient, and/or Mental Health social work experience.
  • Oncology setting preferred or community setting Age-specific or behavior-specific knowledge and experience in working with specific patient populations.
  • Oral and written language skills, visual and aural skills to effectively communicate and provide for patient needs.
  • Accountability for professional practice, commitment to patient care, and commitment to uphold the Patient Bill of Rights.
  • Current CPR Healthcare Provider certification is required at date of hire.
Desired Qualifications
  • 3 years of experience working in oncology, hospice, or grief services.
